Counseling Referral Form

Please use this form to see the school counselor. Teachers, students & parents can make a referral.  We will schedule a visit as soon as possible. This document is NOT for emergencies or crisis interventions. Call an administrator or counselor immediately for crisis situations.

Bullying Reporting Form

Bullying is unwanted, aggressive behaviors among school aged children that involves a real or perceived power imbalance. The behavior is repeated, or has the potential to be repeated over time.

Key elements of bullying in a school setting include:

 

1. Aggressive Behavior: Actions that are intended to cause harm or discomfort to another student.

2. Power Imbalance: The bully uses their power—such as physical strength, access to embarrassing information, or popularity—to control or harm others.

3. Repetition: Bullying behaviors happen more than once or have the potential to happen multiple times.

4. Impact on the Victim: The victim of bullying may experience physical, emotional, or social harm, which can affect their well-being and academic performance.
